Advance Directive

An Advance Health Care Directive allows you to name a trusted person to make decisions concerning your medical care should you become incapacitated. You will select a Health Care Representative and an Alternate Health Care Representative. Your physician will follow the directions of your Health Care Representative if you are unable to communicate with your physician. In an Advance Directive, you also state your wishes regarding your health care treatment.
